Before Your First Visit
Fill out Health History Form
These forms will be sent to you digitally through the secured CHARMED HEALTH online patient portal.
First create a profile then fill out the health history questionarre and the office policies and procedures form. These enable both parties to properly prepare for the first visit.
Gather any relevant med docs
Relevants medical documents might include labs, blood work, imaging, or pharmaceuticals. If you can’t locate them, don’t worry, they can be helpful but not necessary.
Submit insurance info (ACU only)
If you are using medical insurance to cover acupuncture treatment, please submit a copy of your insurance at least 24 hours before your first visit to ensure verficiation of benefits.
Select Proper Clothing (ACU only)
For those receive acupuncture, wear loose fitting clothing so that your arms and legs below the knees and elbows are accessible. I suggest patients dress as if they are going to a light exercise class.
At Your First Visit
What to expect in a first office visit
First visits are very thorough. I allot 90 minutes for this time which always seems to fly by. We go through your complete health history to look for root issues impacting your health.
In holistic health, everything is connected – your emotions impact your physiology and vice-versa so I am interested in getting to know you as a whole person; this in-depth process will enable me to best support you and set a strong foundation to work off in follow-ups.
Home office setting
The office is located adjacent to a home resident. The office has a separate entrance which will be the first door to the right as you go up the walkway
Parking Info
Street Parking is available diretly in front of the office on Meetinghouse Rd as well as in the commercial lot on the corner of Meetinghouse Rd and Old York Road
